SONQUEST RAINFOREST - Registration Form
Faith Tabernacle is hosting a week-long Vacation Bible School for Explorers ages 4-12 years old.
Enter a world of AMAZING animals, FABULOUS fish, & INTELLIGENT insects -- we've travelled a long way upriver to discover this abundant rainforest! SonQuest Rainforest is a vibrant, dynamic place filled with enormous plants & the sounds of lively birds & animals! 5 Days of Discovery not only give children the knowledge of God's Word, but also the opportunity to respond to His truth by living out their Christian faith.
** EARLYBIRD REGISTRATION: Sunday, June 28th thru Sunday July 18th (REGISTER EARLY for a chance to WIN a cool prize!) Registration forms will be available ONLINE (here) & at the church on the 'VBS table' or through your child's SUNDAY SCHOOL &/or JUNIOR CHURCH teacher(s). Contact Donna Harris to send flyers & registration forms to your family, friends & neighbours. Please RETURN forms ASAP ONLINE (here) or to the 'Registration Box' on the 'VBS Table', or to Donna Harris (or your child's SS or JC teacher). If you would like your child(ren) {up to 3 children can be registered on one form} to participate in this event, complete and {sign} this following statement of consent and release of liability. As a parent or legal guardian you remain fully responsible for the actions & conduct of your child, including any legal responsibility which may result.
PARENT PERMISSION & MEDICAL RELEASE FORM
I (we) hereby consent to participation by the under-written child(ren) in this activity. I understand that this event will take place on the church facilities and that my child will be under the supervision of the designated person on the stated dates. I further consent to the conditions stated above regarding participation in this event. In consideration of my child's being allowed to participate in this Vacation Bible School, I agree to indemnify and hold harmless FAITH TABERNACLE OF LONDON and it's representatives, including leaders & volunteers, from any and all claims, including negligence, arising from or relating to my child's participation in this event. This indemnification and hold-harmless agreement does not apply to claims for intentional misconduct or gross negligence.
I (we) the undersigned parent/guardian of the child(ren) stated below, a minor(s), do hereby authorize adult volunteers of FAITH TABERNACLE of LONDON as agent(s) for the undersigned to consent to any medical or surgical care deemed advisable by any accredited physician or surgeon in an approved emergency clinic or hospital. I further release from any liability FAITH TABERNACLE of LONDON, any of its ministries or leaders in the event of an accident in route, during and returning from the above mentioned event. This agreement does not apply to claims for intentional misconduct or gross negligence.
